Terms of Service

By accessing or using FormRx.ai ("the Service"), operated by Aether Practice Solutions Inc. ("we," "us," or "our"), you agree to be bound by these Terms of Service. If you do not agree, do not use the Service.

FormRx.ai is an AI-powered clinical document processing platform. Services include:

• Medical form filling (disability, FMLA, workers' compensation, and other forms)
• Clinical chart summarization (categorization of outside records for pre-visit review)
• Medical letter generation (ADA accommodation, return-to-work, and other physician letters)
• Fax inbox and outbox for receiving and sending clinical documents

The Service is intended for licensed healthcare professionals and their authorized practice staff in the United States. By using the Service, you represent that you are authorized to access patient health information in the course of your professional duties.

You must provide accurate, complete information when creating an account. Each practice should maintain one organizational account. Physicians using the Service must provide a valid National Provider Identifier (NPI). You are responsible for maintaining the confidentiality of your account credentials.

• Base subscription: $59 per month per provider (first physician)
• Additional physicians: $39 per month each
• 14-day free trial included with new subscriptions
• Credit card is required at signup
• Billing begins automatically at the end of the trial period
• All fees are in U.S. dollars and are non-refundable except as described in our cancellation policy

• You may cancel your subscription at any time from your Billing page or by contacting admin@formrx.ai
• Cancellation takes effect at the end of your current billing period
• You retain full access to the Service until your billing period ends
• No partial month refunds are provided
• Uploaded documents are retained for 90 days. Structured summaries and extracted data are retained for the duration of your subscription.
• After cancellation, all data is deleted within 30 days
• Fax numbers provisioned through FormRx will be released 30 days after cancellation

FormRx operates as a Business Associate under HIPAA. Key compliance measures:

• A Business Associate Agreement (BAA) is included with every subscription at no additional cost
• All Protected Health Information (PHI) is encrypted at rest using AES-256 and in transit using TLS 1.2+
• PHI is processed exclusively within AWS services covered by the AWS BAA
• Uploaded documents (PDFs) are retained for 90 days. Structured clinical data (summaries, form mappings, letter content) is retained for the duration of your subscription
• After cancellation, all data is deleted within 30 days
• Full audit logging of all access to clinical data
• 15-minute session timeout for HIPAA compliance

FormRx uses artificial intelligence to extract and process clinical data. All AI-generated output — including filled forms, chart summaries, and letters — is preliminary and must be reviewed by a licensed physician before use. FormRx does not provide medical advice, diagnosis, or treatment recommendations. The signing physician is solely responsible for verifying the accuracy of all clinical content.

FormRx strives for accuracy but does not guarantee error-free output. The reviewing physician is responsible for all clinical content in signed documents. FormRx is not liable for clinical decisions made based on AI-generated summaries, form fills, or letters.

Clinical data uploaded by users remains the property of the user and their practice. FormRx does not sell, share, or use clinical data for any purpose other than providing the Service. We do not use your data to train AI models.

FormRx.ai, the FormRx logo, and associated branding are the property of Aether Practice Solutions Inc. Users may not reproduce, distribute, or create derivative works based on our branding or proprietary technology.

We may suspend or terminate accounts that violate these terms, engage in fraudulent activity, misuse the Service, or fail to maintain current payment. You may terminate your account at any time by canceling your subscription.

To the maximum extent permitted by law, Aether Practice Solutions Inc.'s total liability for any claims arising under these terms shall not exceed the amounts paid by you in the 12 months preceding the claim. We are not liable for indirect, incidental, special, consequential, or punitive damages.

These terms are governed by the laws of the State of Washington, USA, without regard to conflict of law principles.
